The New York City Department of Health first flagged the cluster on July 4, 2026, with just 14 confirmed cases tied to the Upper East Side. Within days, that number climbed past 20, and city officials expanded their investigation zone to cover more zip codes.

By mid-July the case count had jumped to 36, then 59, then 60, as health workers tested more rooftop cooling towers across the affected area.

By late July, the outbreak had grown even larger. A fourth death was confirmed on July 21, with officials saying they still believed the source of exposure had been eliminated.

Days later, health officials confirmed a fifth death, with the case count reaching 82 and eight people still in the hospital. City health teams have continued watching for new cases even as the pace of new diagnoses appears to be slowing.

Why Cooling Towers Keep Causing These Outbreaks

Legionnaires’ disease is a severe form of pneumonia caused by Legionella bacteria, which grow in warm water. People get sick by breathing in tiny water droplets, or mist, that carry the bacteria.

Rooftop cooling towers, which help cool large buildings, are a common breeding ground because they hold warm water and can spray fine mist into the air above city streets.

Health officials stress the disease does not spread from person to person. You cannot catch it from being near someone who is sick, and you cannot get it from drinking water.

New York City’s own surveillance data show most cases in the city are not linked to travel or hospitals at all. Instead, they trace back to everyday environmental sources like cooling towers, decorative fountains, or misting systems outside grocery stores.

City Officials Push New Testing And Response Rules

In response to the growing cluster, the Mamdani administration announced new strategies aimed at quickly finding and shutting down infected water systems. Crews have been sampling cooling towers across the investigation zone, testing water for the bacteria before buildings are allowed to keep operating them.

Officials have also flagged concerns about a separate cyclospora illness linked to contaminated produce, adding pressure on the city’s public health teams this summer.

This kind of layered response mirrors how the city handled a nearly identical outbreak in Harlem just one year earlier. That cluster grew to 114 confirmed cases and seven deaths before investigators used genetic testing to match bacteria samples from patients to two specific buildings, including Harlem Hospital.

Health officials there eventually declared the outbreak over once testing confirmed the towers had been cleaned.

A Pattern New Yorkers Should Recognize By Now

Cooling tower outbreaks are not new to New York City. Research on prior clusters, including one in 2015, shows investigators repeatedly rely on genetic matching between human and environmental bacteria samples to pin down a source.

That process takes time, which explains why case counts and death tolls tend to climb for weeks before officials can say with confidence that a specific rooftop system caused the spread.

Health experts note the risk to most residents remains low, since Legionnaires’ disease requires inhaling contaminated mist rather than casual contact. Still, officials urge anyone with flu-like symptoms, cough, or shortness of breath in the affected zip codes to seek medical attention quickly.

Early treatment with antibiotics greatly improves outcomes, but delays can turn a treatable pneumonia into a fatal one, as this summer’s rising death toll makes painfully clear.
